Consent to the Processing of Personal Data

I, acting freely, of my own will and in my own interest, and confirming my legal capacity, hereby give my consent to A-MEDIA LLC (ООО «А-МЕДИА», INN 9710096078, OGRN 1227700102321, address: 108814, Moscow, Kommunarka municipal district, Kommunarka settlement, Bachurinskaya st., 7, bldg. 2, premises 4K/12; hereinafter the “Operator”) to the processing of my personal data on the following terms.

1. List of Personal Data to the Processing of Which Consent Is Given

— surname, first name, patronymic (if any);

— contact telephone number;

— e-mail address;

— name of the organisation and position (if specified by me);

— information contained in my request (description of the task, project, services of interest);

— other information specified by me independently in the form on the website epic-name.ru (hereinafter the “Website”).

2. Purposes of Processing Personal Data

Consideration of my request (enquiry) submitted via the form on the Website; contacting me using the contact details I have specified for consultation on the Operator’s services, discussion of the task, preparation and approval of a commercial proposal and the terms of a contract; sending me information directly related to my request (enquiry).

3. List of Actions with Personal Data and Methods of Processing

Collection, recording, systematisation, accumulation, storage, clarification (updating, modification), extraction, use, transfer (provision, access) to persons processing personal data on behalf of the Operator on the basis of contracts concluded with them: Beget LLC (ООО «Бегет», INN 7801451618, St. Petersburg) — hosting provider; 1C-Bitrix LLC (ООО «1С-Битрикс», INN 7717586110, Moscow) — operator of the Bitrix24 cloud service; blocking, deletion, destruction. Processing is carried out both with and without the use of automation tools.

4. Term of the Consent

The consent is valid from the moment it is given until the purposes of processing are achieved, but for no more than 3 (three) years from the moment it is given, or until it is withdrawn, whichever occurs first.

5. Procedure for Withdrawing the Consent

The consent may be withdrawn by me at any time by sending a statement to the Operator by e-mail at info@epic-name.ru or by post to: 108814, Moscow, Kommunarka municipal district, Kommunarka settlement, Bachurinskaya st., 7, bldg. 2, premises 4K/12. In the event of withdrawal of the consent, the Operator ceases processing the personal data and destroys it within a period not exceeding 30 (thirty) days from the date of receipt of the withdrawal, provided that there are no other legal grounds for processing under the legislation of the Russian Federation.

6. Final Provisions

I confirm that the personal data specified by me belongs to me personally and is accurate. The rights of the personal data subject provided for by Federal Law No. 152-FZ of 27 July 2006 “On Personal Data” have been explained to me. I have read the Operator’s Personal Data Processing Policy posted on the Website.

The consent is given by me by ticking the relevant box (checkbox) in the form on the Website, which, in accordance with Part 1 of Article 9 of Federal Law No. 152-FZ of 27 July 2006, is a form that makes it possible to confirm the fact that consent has been obtained.
